Older Adults Home Treatment Team provide short term intensive home treatment to older adults with a functional mental health problem or dementia diagnosis.

Older Adults Home Treatment Team work closely with other community and hospital-based teams – as well as with any support network of carers, friends and/or family.  Their aim is to assess and treat people at home and prevent admission to a mental health ward where possible. They also try to enable earlier discharge from an acute inpatient mental health ward back to their home when additional mental health support has been identified.

Referrals are accepted from the Older Adult Community Mental Health Teams, Sheffield Memory Service, Liaison Psychiatry Service and other specialist services within Sheffield Health and Social Care Trust.  They don’t accept self-referrals but if you feel that you or a loved one may benefit from their support you can speak to your GP and request a referral to the Older Adults Community Mental Health Team.
